Need your guidance in building the below format report.

The data for the First Four Parameters I am getting from the QVD file. The last to parameters are expected to be as input fields. User can input the estimated Sales and Qty. This will help calculate the estimated sales growth %.
To come up with this report, which chart, Pivot or Straight table should be considered, so that I can i have last two rows with input field?
