Seek ver 11 and 9 QV user to help on check my QV doc

I need some one to help me check , if you open my QV doc , are you see the same as below image ? and let me you are using ver 9 or 11 pls.

After i upgrade my QV ver 9 from SR3 to SR7 it work fine now. i see what you see.
