Try this in Conditional Show:

vShow = '%CHF Patients on Pathway' (Between single quotes)

Create a variable as below:

vShow = [%CHF Patients on Pathway]

In Conditional Show enclose with square brackets:

vShow = [%CHF Patients on Pathway]

No It's not working chart will not display.
