The difference is that Windows takes care of the file permission in NTFS. If you look at the QVW you would see the users that have rights to the document under the security tab. QVS will check these rights before it will allow a user to access the file.
In DMS mode QVS keep a list of who has rights to a QVW and that is stored in the .meta file.
