Find Source and Destination based on visit sequence

!Greetings.

Dear All,

Attached is a sample dataset of planned shops visit.

1st sheet (Shops) comprises of all shops and shop names.

2nd sheet (Shops visit Sequence). One shop to another

3rd sheet (Visit Plan) comprises of daily visit plan (by week and by day) 1= Yes, 0 = No

I need help in deriving a new fields "From Destination" and "To Destination" on the bases of shops visit sequence .

For eg.

PlanID	Visit Sequence	From Destination	To Destination
1	1	1001	1002
1	2	1002	1003
1	3	1003	1004
1	4	1004	1005
2	1	1004	1005
2	2	1005	1003
2	3	1003	1002

Any Quick help would be appreciated.
