Header in Pivot table - using Pick and Valuelist

I need to format the table as below:

I can use Valuelist for Cost Per Quantity and Discount per quantity . But how to include the expression for sales ,Discount and quantity to get the format as above

Valuelist  shouldn't be necessary here.
