Map only bringing in first line

I'm having a strange problem where when mapping it is only bringing in the first value for the table and assigning that to all the matches it finds.  I have included a screen shot.  Has anyone ever run into this?

A mapping approach is 1:1 mapping values. If you need 1:n, you'll need to join.
