QVA is undefined

I'm looking for a way to diagnose my server setup because as it is, I have not found found anything out of place.

There are several open/closed questions that insist the ApplicationPools must be run by the correct user but I've checked this twice over.

I'm using QV SR 11, IIS 7.5 and the application that displays the "QVA is undefined" is a combination of ASP.NET/QV Workbench
