Heat map-create a new field(of existent fields)

Hi guys,
I'm a new in QLIK SENSE, My table structure (image 1) contains 12 symptoms (fever, cough,headache,...) and all of them are text data type where '0'=doesn't have this symptom or '1'=have this symptom. I'd like to set a heatmap betwen date and these symptoms. The fact is a heatmap structure just allows me put 2 dimension and 1 measure. I have 12 fields of symptoms, i'll have to create a new field as an array or list called "sympthom" and envolve all these 12 symptoms in one field.(This is my idea but i cannot get it until now).

I tried to simulate it, so, i set fixed values (image 2,3), but i cannot do with real time data, because the editor doesn't allow to use functions or conditionals.

Could you give any ideas please?
