login issue for QMC

hello every one,

to day i changed the authentication mode of my QMC from windows authentication mode to forms by seeing at https://community.qlik.com/thread/160788

but after doing so, i was logged out from my QMC and when tried to login to QMC it showing the white blank image, i am attaching the screenshot of it.

please help to rectify it.

You're logging on as a different user then I think. Does that user have a login token assigned?
