Is this the correct way to do lookup on id with multliple fields of same table or do I have to take the same table input multiple times and do lookup.

If both Indicator__c and Donor__c can be used to join with the Id column on the lookup table, then you'll need to add a second lookup using the same input as row8.

Then, for each lookup you use a different column from the main row.
