Best Month in a rolling 12 month period

I'm trying to get the top data set into the bottom format so I have one column identifying when the best month for a salesperson was in the last 12 months, the next column tells the amount of that best month and the last tells whether or not the last month was their best month. 

Would appreciate some help with this one, haven't been able to get to this using other posts.
