How to set header for combination of two or more columns in pivot table

eg

   Weekly  Sales           Monthly Sales

---------------------------------------------------

  Tons     Amount       Tons      Amount

Either create a dimension with the values Weekly Sales and Monthly Sales or use text boxes as labels. See attached example.
