Loading Information from Forms

Has anyone experimented or perfected a way to load "Form Data" into Qlikview?

I have a Quality Assurance form that I would like to load into qlikview to manage the aggregate data. (See Below)

The problem is, that the form was designed to be input friendly, but any analysis has been done manually to this point--copying values into another excel sheet to make charts and such.

Does anyone know a way to load information from specific fields (cells) within a sheet, while maintaining the relationship between those cell, and loading multiple identically formatted sheets?
