Looking for the QVW for user custom creation of pivot table

There is a document somewhere with example of a user customizable pivot table.  Can anyone point me to it? 

It contains a list of the fields available and allows the user to select, via check boxes I believe, which fields to include as rows and columns as well as which metrics to include in the pivot object.
