I'm new to the community so apologise if this is the inappropriate forum or terminology.
We have recently started using Qlik in a clients organisation, we have a document published to the access point which we would like users with licences to be able to manually refresh upon a trigger.
My question is if a user with a document CAL accesses the document and triggers a refresh which account does the server use to run the query?
Would it be the user or the Qlik service account?
Any help would be appreciated,

All the jobs triggered on the server, will run as services and use the account that's registered to be used for the services. It's never the actual "end user" account.

Depends on how you trigger the refresh and exactly how the refresh is performed.
If you use a user triggered EDX event (or the Qv management console) to reload the job, the reload will run under the QV server or publisher service account. The same applies to a regularly (Publisher) scheduled event.
If you allow users to load the qv document into the desktop, via a file share (not open in server), then the reload will be executed on the user's machine under their user account. You can disable this selectively by limiting access through a share, and/or by applying section access and appropriate document security settings.
