For each chart dimension use Date(Date,'MMM-YYYY') to get the format you displayed a la "Mar-2017", et al.
Each chart should have 3 expressions:

Avg(Healthy)
Max(Healthy)
Min(Challenge)
Avg(Challenge)
Max(Challenge)
Min(Danger)
Avg(Danger)
Max(Danger)
Min(Trending)
