Requirement Capture Form/Doc

Hi,

Does anyone have any standard form/document to capture requirements from users?

What I am looking for is a form/document that asks users some general information about the application they have requested to build. eg: Business Requirement, Sample report, Target audience, expected number of users, etc.

Any related document will be of great help.
