User-created message board with uploads in QV

Hello, I'm trying to create a login-based QV document with the following functionality:

• A QVS user with administrative privileges logs in
• A single "Submit" button will allow the user to simultaneously upload a file (.pdf etc.) and enter a text description of the file
• Upon submission, the file is uploaded to a directory, and a table row is generated in .xls or .csv with the following fields:
  • File description, directory path/URL to the uploaded file, and submission timestamp as MM/DD/YY HH:MM
• The .xls or .csv table is updated upon each submission, and is available for querying in QV

I neglected to provide the login credentials from the sample .qvw:

Administrator: username: admin , password: password

User without upload privileges: username: user , password: user
