It's still available on the Partner download site, if you are not a partner, you need to contact your local Qlik representative.

PS v12 is actually up to SR 16

The upgrade from within major release versions is very simple - you basically just run the installation .exe file for you the newer version. You still should, of course, back up the pertinent files and directories.
