QV Server version up gradation(Urgent)

Hi Team,

             We want to upgrade QV server edition from 11.00.11154.0 IR 64-bit Edition(X64) ->To some new Service pack Release(like SR6 11.20).

So what precautions we need to take before upgradation

(1) Is there any backup need?(QVDs,QVW's)

(2) Is there any CAL'S need,

(3) If i did upgrade again Scheduling in QMC is required freshly  for all reports or the old scheduling process is saved?

I don't know all questions please explain if any other precautions and how can i upgrade, if it possible can any one share screen shots for upgrade.

In advance thanks for u r valuable help.
