convert part of records to columns

I have a table like the below screenshot

that I have to transform and load in a table with the below structure.

Can someone help me out?

Why donot you try to use taggregaterow with group by on 1st 3 cols and the min and max of next two cols.

But above will work if you have maximum 2 records having same 1st 3 cols .
