Create your own diagram

Hello qlikers,

I want to create a own diagram, but I don't know how to create this diagram.

The diagram should look like:

This is the raw diagram, also without datas.  I get the datas from an excel file, the excel file looks like:

So, the diagram at the end should look like:

So my question is, how can I create a own diagram? Is there something like forms in MS Office word?
