calendar with multiple dates fields

hello all i am build an app that has multiple dates fields and i would like the end users to be able to select specific month which would apply to date fields.

for example 

tableA:

load

key,

xxxx,

aaaa,

orderdate,

wqq

from xyz;

Tableb:

load

key,

bbbb,

cccc,

comitedate,

yyu

from abc;

Tablec:

load

key,

dddd,

eeee,

deliverydate,

eee

from def;
