You have a number of challenges here which may be very difficult to overcome with script without first formatting the source data.
For instance your field names are duplicated unless you combine them with the line above.
Personally I would either change the data extract at source (presuming you have a database) or failing that I would create a second sheet and by using excel lookup functions create a more readable source file.
