Custom Report Format

Hi All,

I want to create Custom Report based on which is the attachment document. Kindly please help me.

I want to know hoe they created below tables.

My guess is that these two tables are created as an Inline tables or loaded through Excel file... I mean they don't have to be... but that's how they are done most of the time.
