You go to the directory where QlikView is installed anf type qv.exe myapp.qvw

Search for command line in regular QlikView Reference Manual and you find the parameters it supports.

Open it in which way?

In QV desktop? Just double click the QVW file.

In QV desktop from a bat file:   C:\Program Files\QlikView\qv.exe C:\Models\MyModel.qvw

     To reload using desktop from a bat file:   C:\Program Files\QlikView\qv.exe /r C:\Models\MyModel.qvw

To reload in server: Using EDX in QlikView 11
