Survey Feedback Form in Qlik Sense

Hello ,

I have a Qlik Sense application where I need to ask the users to fill a feedback form and store it in somewhere ( DB or QVD)

Now, I am able to create a logic to fill something and store it in QVD but I need to create a feedback form and embed it to the qliksense apps.

How to achieve this ?
