Examples of a form to gather enhancement or project requests from users?

Hi all

I'm new to qliksense and found the Qlik in the Enterprise - Center of Excellence - 1.3.docx‌ very helpful.

There is a section on "project intake" e.g. how to handle enhancement requests from users.  It has some useful bullets, but wanted to know if anyone could share some more detailed examples they might be using in their organizations?
