UDC - LDAP advanced - problem with connection

Hi,
I need to connect to AD by downloading additional fields from AD, unfortunately there is no such in the standard connector. So I decided to use Advanced LDAP and this is where the problem begins:
1. I don't want to connect anymore without setting additional fields 
2. Is it enough to set the selected fields in the custom attributes field?

What else do I have to set in the connector to connect?
Below is a screeen with what I have set and they are not working at the moment, any suggestions?
