Wile reloading session monitor App getting (404) Not Found error

Hi Team,
While reloading session monitor app i am getting 

(404) Not Found error.

I have added port in to connection string of monitoring apps. but still error is same.

please help me to solve this issue

Make sure the assigned user is a rootadmin
