We have in the Operations Monitor the same task (same ID) but with 2 different names. One is "Manually triggered reload of <app>" and the other is a custom name than the admin had given: "TASK01_<appname>".
All the charts duplicate the same data for both applications and it is like if there where 2 tasks with the same reload schedule, etc.
The app is reloading and working ok.
We think that the solution is to delete the task and create a new one.
Have you experienced this behavior? Is it expected or is it a bug in the Operations Monitor?
