Qlikview server lost all setting in QMC

Hello,

This happened to one of our clients one time recently: all settings were lost in Qlikeview Management Console.

we have set up reload task on QMC, because the settings were gone, client could  not open dashboard, getting error 'failed to open document, file not found'. We have to manually reapply all settings back.

Client claimed they did not do anything like applying micorsoft patch/updates, maintenance etc..

Can somebody explain for us why this happened?
