Best way to document changes within a application

Hi Everyone!

I have more of a general question that has to do with change control documentation. I am currently trying to brainstorm some different ways to document all the changes that happen to an application over its life cycle and was wondering what everyone had the best luck with? I currently document all the changes made to applications, but it is a little messy! Does anyone have a go to template they use for their change control documentation for Qlik apps? 

I appreciate any feedback!

Outside of an official change management suite, what I have seen most customers do is use the Load Scrip comments to document changes there and for visualizations, maybe hidden sheets related to history or an actual tab that does contain change history, just a matter of whether you want users to see things or not.  We also have the prj files, but I do not think there is anything there that is going to allow you to document anything.
