What would be the best approach to do this?

Hello,

I have three fields in a straight table called Policy_Name, FirstName and LastName\

When you click on a certification it will show the policy violations a person did. And a certification can be activated any number of times a certifier wants. Sometimes, once a year , sometimes however many times a policy violation is detected. I want to add an extra column that will say Yes or No. Yes if a policy violation exists before for the given person, no if otherwise. Right now , you have to select a year and month along with the certification. But  I can bring in all the policy violations for every certification without selecting the year, month and certification. I can probably also bring in the date the policy violation occurred.

What would be the best approach to do what I want? Has anyone done anything similar? I would really appreciate the help. Thanks.
