AND and OR in expression

Hi, I'm trying to get IDs with certain Zip codes (123,456,789,1234,9876) and claim codes (0011,2234,4435) or CPT codes (98675,96745,90876,89765)

Basically, I need IDs with all zip codes but either of the claim codes or CPT codes.

Please advise thank you.
