Qlik Sense - Field Name Not Found Error

[Weekly Report]

LOAD

"Contact Type"

"State"

"City"

plus other fields ….

FROM [lib://Master/sdut/WeeklyReport.csv]

(txt,codepage is 28591,embedded labels, delimiter is '\t',msq);

or using (txt, codepage is 28591,embedded labels, delimiter is '|',msq);

either method is not working..

Please advise.  Thank you!
