Basic Extension Documentation and Template

Based on a lot of requests, I wrote a piece of informal documentation (found here), and also created a template extension meant to be used as a starting point (found here).

Again, I put this together to be a starting point for the most basic of extension topics.  I think I covered everything of what would be needed for the most common extension uses, but if you feel like I missed something, please let me know.
