Salesforce Connector Pointed to Custom Domain

I am trying to stand up a Salesforce Connector in Qlik Sense, but I keep running into an INVALID_LOGIN error. Our instance of Salesforce lives on a Custom URL so I think it is trying to connect to me the normal Salesforce page which is why our connection is failing. 

Is there a way to setup our Salesforce Connector to point to a custom URL? I've attached screenshots of what our Connector Setup Screen looks like. It doesn't really match the documentation so I'm not sure if we are on an older version? If so -- how do we update?
