How to we implement Pivot

Hi team,
I have 7 columns in my input .. I need to map  card ,port and rote under name
i.e as shown below tables Input 7 column output 5 column
Card ,port, rote data I need to map it under name ( I mean all the values should come under name In row format )


requirement is I nede to map 3 input column data in single column ( all the data from 3 input column reflex like 1 column data in output)
I did it in datastage using pivot enterprise but here I m not sure how do I do it . Please help me with logic
