Our Salesforce TEST connection in Studio under meta data is successful most of the time when testing it. However, when the connection is in context mode, the tSalesforceOutput component fails every time. The salesforce PROD works fine, both in meta data and in context mode.
Any suggestions how to resolve this issue is greatly appreciated.

Could you please clarify in which Talend version/edition you are?
From your screenshot QA connection, the URL given in the connection uses API 39 and you are using a test Salesforce account.
Are you sure about the version and the test Salesforce account credential?

Yes, we are sure the test SF account credentials are correct because as noted before, the SalesforceDev1 connection for QA (using the SF test account) under metadata works fine.
Since the SalesforceDev1 connection is successful, then the SF credentials are correct right?
Do the images help in understanding what the issue is?

As there is time difference we are not able to respond promptly to your question. Have you already created a support case on talend support portal? In this way, our colleagues from support team will give you a remote assistance through support portal with priority.
We have added support 39.0 starting from 6.4.1.
From the error message, so far it only tell us something wrong with the login.(password, username, security token...)

Do I need to setup a trial for accessing the support portal? The whole process is very confusing.
Again, we have a Talend subscription and need TECHNICAL SUPPORT.
We are going around circles and no progress has been made.

Don't be frustrated. Please do this thing step by step.
Firstly, are you able to access this page:
https://login.talend.com/support-login.php
Please login with your Support Portal account. Is there any issue for your support portal account? It should be given to you by your talend representative when you get the subscription solution.
Any issue about the login issue, please send an email to Community <community@talend.com> and our manager will check your valid email address and username to address your login issue immediately (I'm supposing that you are missing your support portal account).
Do you have any contact of your own talend representative? Name or email address? I will help you to get his number for you if possible.
