Generic load

I am trying to pull a generic load but unable to do. Please help me. I have attached 2 files with the mail, please check and advised.

You have shown a formatted picture. Is that data in a file, like csv or excel? What are the column names?

Thanks for the reply. Yes, it is a data and column names are given in the left side, lik date , time.

What I meant was does the file have a column name for those date/time/location things. So you can do something like:

Generic Load AccidentId, Heading, Detail 
From ...

Well it is not a file. I took a screenshot of a screen and copied it here. 

I am trying for a certification and was looking to work on generic load and found this as a perfect example.
