QvsAdminDataProvider on QV12?

Has anyone used the QvsAdminDataProvider.dll connector with QV12? It fails on all SRs I've tried.  Anyone else tried it and had problems or success?  The error I get is:

Custom connector error

Custom connection failed

SQL SELECT *

FROM Collaboration

Same problem here! Running QV12.10SR2

If I try to create a new connection I get
