multiple lines in Line Chart

Hi,

I am back again with a very basic question. Following is the simple table with 3 columns with monthly %age data for each region (Its a result from Pivot). I want to make a line chart which shows Separate line for each region with values shown on the points. Something like in the attached image

What i am getting is that the line is joind throughout between the regions.
