report format: reflect the table fields as a header

My data looks like this :

type	year	quarter	week	count
assessment	2016	Q1	57	500
bridging	2016	q2	58	6000

I need it to look like this instead

name	2016	jan	quarter	week 57
bridging	5000	5	60	1
assessment	15000	15	800	15

Any guidance will be deeply appreciated please

Is it mandatory to display the result in table(Straight/Pivot) chart or any other combination of Objects is also OK?
